Authorizes rural health facilities (hospitals, behavioral health facilities, clinics, mobile clinics, home health agencies, and long-term care facilities) to use USDA community facility loans under sections 306(a) and 310B of the Consolidated Farm and Rural Development Act

Permits funds to be used for refinancing debt, updating telehealth and equipment, online databases, and supporting operating expenses and reserve funds

Requires assistance to preserve rural health access, meaningfully improve the facility's financial position, and meet Rural Development Agency feasibility and security requirements

Allows the Secretary to waive certain requirements for facilities that are insolvent or located in persistent poverty areas (20% poverty rate for 30 years), socially vulnerable communities, or distressed areas

Defines rural areas as populations of 50,000 or fewer not adjacent to cities over 50,000; takes effect 6 months after enactment
