Amends Section 477 of the Social Security Act to add a new purpose: connecting foster youth in eligible families with evidence-based home visiting and support services provided under the Maternal, Infant, and Early Childhood Home Visiting Program (Section 511)

Requires state chief executive officers to certify that processes exist to inform foster youth participants who are expectant or parenting about available evidence-based home visiting services in their state

Authorizes states to use their federal allotments to provide tailored case management and resource coordination services specifically for expectant or parenting youth eligible for foster care programs

Takes effect 1 year after enactment, applying to state plans approved by the Secretary of Health and Human Services on or after that date
