Requires the Center for Medicare and Medicaid Innovation (CMMI) to test the "Specialty Health Care Services Access Model" to improve access to specialty care through telehealth and remote technologies for Medicare, Medicaid, and CHIP beneficiaries in rural and underserved areas

Provider networks must include at least 50 Federally qualified health centers, rural health clinics, critical access hospitals, or rural emergency hospitals, with at least half located in rural areas

Participating networks must be 501(c)(3) nonprofit entities with established records of supporting health care delivery in rural and underserved communities across multiple regions

Eligible individuals include those entitled to Medicare Part A or enrolled in Part B, or enrolled in Medicaid or CHIP, who are located in rural or underserved areas as specified by the Secretary

Appropriated funds are subject to requirements under Public Law 117–328 for programs authorized under sections 330–340 of the Public Health Service Act
