Requires the Center for Medicare and Medicaid Innovation to test a model eliminating copays, coinsurance, and deductibles for opioid use disorder treatments, overdose reversal medications, and recovery support services in 15 states with high Medicare populations, high opioid overdose deaths, and significant rural areas within 6 months of enactment

Mandates group health plans and insurers provide coverage with no cost-sharing for prescription drugs treating opioid use disorders or reversing overdose, behavioral health services including residential treatment, and community recovery support services like peer counseling and transportation, effective January 1, 2027

Increases the federal Medicaid matching rate to 90% for state expenditures on medication-assisted treatment for opioid use disorders

Gives states the option to include recovery support services such as peer counseling and transportation as part of Medicaid-covered medication-assisted treatment

Protects the Medicare pilot model from early termination or modification for its first five years of operation
