Adds Medicare Part B coverage for dental services (cleanings, exams, fillings, crowns, root canals, extractions, emergency care, and dentures), vision services (routine eye exams and refractive procedures), and hearing services (routine hearing exams and hearing aids)

Phases in Medicare payment rates gradually over 8 years, starting at 0% in year one and increasing by 10 percentage points annually until reaching 80% coverage in year eight

Limits frequency of covered services: dental cleanings and exams (2 per year), routine eye exams (1 per year), eyeglass lenses (1 pair per year), eyeglass frames (1 set per 24 months), contact lenses ($200 cap per 24 months), dentures (1 full/partial set per 5 years), and hearing aids (1 per ear per 48 months)

Expands Medicaid to explicitly include dental, vision, and hearing services as defined under the new Medicare provisions, with a 90% Federal Medical Assistance Percentage (FMAP) for state expenditures on these services for adults

Grants the Secretary of Health and Human Services authority to apply additional coverage limitations, prior authorization requirements, and modify benefits consistent with U.S. Preventive Services Task Force recommendations
