Permanently extends COVID-19 telehealth flexibilities for cardiac rehabilitation, intensive cardiac rehabilitation, and pulmonary rehabilitation programs under Medicare, allowing services to be delivered in patients' homes via audio-visual real-time communications technology

Allows physicians, physician assistants, nurse practitioners, and clinical nurse specialists to provide virtual supervision of rehabilitation programs through real-time video technology

Removes geographic restrictions for telehealth services related to cardiopulmonary rehabilitation programs, expanding eligible originating sites to include patient homes

Requires the Secretary of Health and Human Services to establish standards within 30 days for designating patient homes as provider-based locations of hospital outpatient departments, consistent with Hospital Without Walls program waivers

Effective date of January 1, 2026 for all amendments and newly established standards
