Requires each off-campus outpatient department of a hospital to obtain a separate National Provider Identifier (NPI) number distinct from the main hospital's identifier, effective January 1, 2026

Mandates providers submit attestations every 2 years certifying their off-campus departments comply with federal "provider-based" status requirements under 42 CFR 413.65

Directs HHS Secretary to establish within 1 year a rulemaking process for reviewing attestations and verifying compliance through site visits, remote audits, or other means

Requires HHS Inspector General to submit analysis and recommendations to Congress by January 1, 2030 on the effectiveness of the attestation review process

Applies to off-campus departments not located on the hospital campus or within the specified distance from a remote hospital facility location
