Adds physical therapists and occupational therapists as authorized health professionals who can provide falls risk assessments and fall prevention services during Medicare annual wellness visits

Requires Medicare annual wellness visits to include falls risk assessment and fall prevention services for beneficiaries whose physician has determined they fell in the previous calendar year

Adds outpatient occupational therapy and physical therapy services to the initial preventive physical examination (Welcome to Medicare visit) for individuals with a physician-documented fall in the prior year

Changes take effect for visits furnished on or after January 1, 2026

Requires the Secretary of Health and Human Services to submit annual reports to Congress beginning January 1, 2027, on falls experienced by individuals aged 65 and older who received treatment for fall-related pain or injury
