Requires Medicare Part D cost-sharing for chronic care drugs to be based on the "net price" (after manufacturer rebates and discounts) rather than the higher list price, effective January 1, 2027

Applies to drugs in specific USP categories: blood glucose regulators (non-insulin), inhaled corticosteroids, anticholinergic and sympathomimetic bronchodilators, respiratory tract agents, anticoagulants, and other cardiovascular agents

Exempts plans that use flat copayments not tied to drug prices from the net price requirement

Ensures low-income Medicare beneficiaries cannot be charged more than the applicable cost-sharing amount under their plan for chronic care drugs, starting in plan year 2027
