Adds "preventive home visits" as a covered Medicare benefit under Title XVIII of the Social Security Act, allowing beneficiaries to receive home assessments no more than once every 2 years

Qualified professionals would assess home environments, identify health risks, and provide referrals for interventions or home modifications to improve physical function, balance, strength, mobility, fall risk reduction, and nutrition

Visits may be conducted in-person, remotely, or through a combination of both methods

Medicare would pay 100 percent of the cost with no beneficiary cost-sharing, using a bundled payment amount established by the Secretary that includes any referrals made during the visit

Effective date for coverage is January 1, 2027
