Expands Medicare annual wellness visit screenings to include mobility, food security, housing security, transportation access, social support, and other social determinants of health, plus adds balance screening to identify fall risk, effective January 1, 2026

Provides 10% incentive payment bonus to providers who include social determinants screening plus at least 2 additional wellness visit elements during personalized prevention plan services

Expands eligible providers for annual wellness visits to include physical therapists, occupational therapists, and pharmacists, and adds annual wellness visits as covered telehealth services starting January 1, 2026

Requires HHS to conduct a national outreach campaign within 1 year to educate beneficiaries about annual wellness visits, with priority for low-income beneficiaries, rural areas, and health professional shortage areas

Directs CMS to publish a report analyzing annual wellness visit utilization over the preceding 10 years by state and demographics, conduct stakeholder interviews, and perform focus group studies to identify barriers and increase uptake, with appropriations authorized for fiscal years 2026-2030
