Mandates the Secretary of Health and Human Services notify the HHS Inspector General within 60 days when Medicare payments or provider counts in a zip code/county increase by more than 100% in a single year

Imposes identical 100% increase notification requirements for qualified health plans on ACA exchanges, Medicaid, and CHIP programs, with American Health Benefit Exchanges required to annually submit payment data to HHS

Requires the HHS Inspector General to audit any health care program where payments or provider counts in a geographic area increased by at least 400% over a 5-year period, beginning 5 years after enactment

Directs the Office of Management and Budget to issue guidance ensuring recovery of all improper payments and requires agency Inspector General reports to include the amount of improper payments recovered each fiscal year
