Requires the Department of Health and Human Services to provide the vaccination exemption form in an online format and transmit completed forms directly to the relevant school or LEA

Maintains the option for in-person consultation at local health departments, with a fee of up to $25 for that service

Exemption forms may be claimed for medical reasons or personal/religious beliefs, covering required immunizations for students in public and private schools, preschools, child care programs, and kindergartens

Effective date: July 1, 2026
