After June 1, 2025, nondisclosure clauses preventing patients from reporting health care provider conduct to the Division of Professional Licensing are only valid if the provider first gives the patient a model notice about their reporting rights.

The Division of Professional Licensing must create and publish online a model notice informing patients of their option to report unprofessional or unlawful conduct by licensed physicians and osteopaths.

The model notice must include information on what constitutes unprofessional or unlawful conduct, the complaint reporting process, and a reference to Section 58-1-501.

Applies to both physicians (Chapter 67) and osteopathic physicians (Chapter 68) licensed under Utah Code.

Effective date: May 6, 2026; no appropriation required.
