PEHP must rename its existing bariatric surgery pilot program to the "Weight Management and Obesity Pilot Program" and add a GLP-1 medication treatment option beginning in the 2026 plan year

The GLP-1 treatment pathway requires 50% member cost sharing, caps PEHP's monthly pharmacy expenses at $300 per member, and limits coverage to 24 months

Qualified members must choose between bariatric surgery or GLP-1 treatment; those who discontinue GLP-1 within six months may switch to bariatric surgery

Eligibility requires BMI over 40, or BMI over 35 with comorbidities, plus participation in health coaching and demonstrated weight loss aptitude over six months

PEHP must report program results to the Health and Human Services Interim Committee annually before December 1 from 2026 through 2028
