Amends verification forms on signature packets for state initiatives (20A-7-203), state referenda (20A-7-303), local initiatives (20A-7-503), and local referenda (20A-7-603)

Changes the signature collector verification line from requiring the collector's printed "Name" to requiring the collector's "Signature"

Adds two new required certifications for signature collectors on local initiative and referendum packets: that the correct date appears next to each signer's name, and that the collector has not paid or given anything of value to encourage individuals to sign

Standardizes verification form language across all four petition types to ensure consistency between state-level and local-level initiative and referendum processes

Takes effect on May 6, 2026
