Amends Utah Code 20A-6-109 to expand and clarify the types of name variations candidates may use on ballots, including middle names, initials, suffixes (Jr., Sr., I, II, III), maiden names, spouse's last name, parent/step-parent last name, and nicknames

Requires candidates to submit a written request by 5 p.m. on the last day of the declaration of candidacy period to use a name variation on the ballot

Candidates using an alternate surname must submit an affidavit under penalty of perjury confirming the name is one by which they are known in their county of residence

Candidates using a non-common nickname must provide both a personal affidavit and an affidavit signed by five non-family county residents, and the election officer must verify the nickname does not constitute a slogan, imply another person, or provide unfair political advantage
