Exempts transportation services provided through Medicare or Medicaid from these requirements

Transportation providers contracting with medical facilities must supply the required fee disclosure forms; facilities are not required to create forms on behalf of providers

Department of Health must publish the standardized disclosure form on its website by October 1, 2026, with the law taking effect January 1, 2027
