Estimates must include a description of the scheduled service, the provider's standard charges, and any fees for related items or services the patient may reasonably be expected to receive

Estimates must be based on the patient's health benefit plan information, information reasonably available to the provider, and the provider's standard charges

Requires estimates to include a disclaimer stating the estimate is not binding and actual costs may differ based on changes in services or insurance claim processing

Providers are protected from violations when patients or insurers provide inaccurate information or when insurers later adjudicate claims differently than initially indicated

Providers must post written information about patients' right to request estimates in public areas of facilities and on their websites
