Grants adult adoptees direct access to their sealed original birth certificates upon written request and proof of identification to the State Registrar, without requiring a court order or showing of good cause

Creates a contact preference form system allowing birth parents to indicate whether they wish to be contacted, prefer contact through an intermediary, or do not wish to be contacted

Requires the contact preference form to be placed with the original birth certificate and provided to the adult adoptee when they request their records

Specifies that sealed original birth certificates cannot be used for legal purposes, amended, or serve as the certificate of birth in use

Allows birth parents to submit or amend their contact preference form at any time, with previous versions destroyed upon amendment
