Adds required coverage for doula care services, infertility treatment including up to three lifetime cycles of assisted reproductive technology, fertility preservation procedures, and embryo transfer

Requires coverage for hearing aids (up to $1,500 per ear every 24 months) for all ages, expanded from the current requirement covering only children 18 and younger

Mandates coverage for PANDAS/PANS diagnosis and treatment, pasteurized donor human breast milk, and polycystic ovary syndrome treatment

Exempts individual and small group market plans from certain hearing aid and PANDAS/PANS coverage mandates, with those benefits instead incorporated into the essential health benefits benchmark plan to avoid state-funded defrayal requirements
