Requires assessment of 2024 Medicare Community Health Integration service codes (HCPCS G0019 and G0022) and feasibility of establishing Medicaid reimbursement for community health worker services through a state plan amendment

Mandates evaluation of requiring Medicaid managed care organizations to embed certified community health workers in care coordination models for chronic disease management, behavioral health, aging services, and social determinants of health navigation

Calls for developing workforce training pathways accessible to Medicaid and SNAP recipients subject to federal work requirements, coordinated with Virginia Works, the Virginia Community College System, and the Department of Education

Requires DMAS to submit findings and recommendations to the Joint Commission on Health Care and appropriations committee chairs by December 1, 2026
