Directs the Department of Medical Assistance Services (DMAS) to submit state plan amendments to the Centers for Medicare and Medicaid Services to modify consumer-directed services under 1915(c) Home and Community-Based Services Medicaid Waivers

Allows individuals receiving services to serve as their own Employer of Record (EOR) for their service delivery

Permits individuals to designate another person to perform EOR duties on their behalf when they are unable to perform such duties or direct their own care

Requires the Employer Identification Number (EIN) to remain assigned to the individual receiving services, with exceptions for minors, those ineligible to use their existing EIN for tax purposes, or others determined ineligible by DMAS

Authorizes DMAS to limit designation changes to no more than twice per calendar year
