Adopts the Uniform Health Care Decisions Act in Virginia, creating a new Article 8.1 (§§ 54.1-2993.2 through 54.1-2993.31) in Chapter 29 of Title 54.1 to govern advance health care directives and health care decision-making.

Repeals existing Health Care Decisions Act provisions (§§ 54.1-2981 through 54.1-2993) and replaces them with the uniform act framework.

Amends over 30 sections across multiple titles of the Virginia Code to update terminology, changing references from "advance directive" to "advance health care directive" and from "medical decisions" to "health care decisions."

Updates cross-references throughout Virginia law, including provisions related to medical records, abuse and neglect of vulnerable adults, hospital regulations, mental health proceedings, guardianship, and organ donation.

Establishes civil penalty provisions related to compliance with the Uniform Health Care Decisions Act.
