Requires nursing homes to notify residents, families or legal representatives, and the Department when required physician visits do not occur according to the federal schedule under 42 CFR § 483.30(c).

Mandates comprehensive resident assessments within 14 days of admission, after any significant change in condition, and at least annually (every 366 days).

Assessments must cover 14 categories including medical history, physical and mental status, nutritional needs, cognitive status, drug therapy, rehabilitative potential, and advance directives.

Requires nursing homes to review each resident's assessment at least every 92 days and develop a comprehensive care plan within seven days of assessment completion, with copies provided to residents and families upon request within seven days.

Care plans must be coordinated by a registered nurse and prepared by a multidisciplinary team including the attending physician, with meaningful participation opportunities for residents and their families.
