Eliminates the requirement that diabetes treatment be provided by a health care professional under contract with the insurer, allowing patients to receive care from any legally authorized prescriber

Prohibits all cost-sharing requirements for diabetes treatment benefits, including co-payments, coinsurance, deductibles, and dollar limits

Requires health insurers to cover equipment, supplies, and outpatient self-management training and education (including medical nutrition therapy) for all types of diabetes: insulin dependent, insulin using, gestational, and noninsulin using

Applies to health insurance companies, nonprofit hospital and medical service corporations, and health maintenance organizations, but excludes specified disease or limited benefit coverage

Takes effect January 1, 2026 for new and renewed plans, with full implementation required by January 1, 2027
