Authorizes the State Registrar to correct or amend a death certificate to reflect the deceased person's gender identity at the time of death

Allows applications for amendment from certifying health care professionals, medical examiners, hospitals, nursing homes, or immediate family members (spouse, child, grandchild, parent, sibling, grandparent, or guardian)

Requires the Department of Health to adopt rules specifying acceptable prima facie evidence of gender identity, including information from advance directives

Creates a procedure for individuals to file a predesignation of their gender identity while living, which the State Registrar may use when making amendment decisions

Effective date: July 1, 2025
