$450,000 appropriated to the Department of Disabilities, Aging, and Independent Living for a two-year pilot program to enhance community nurse services for aging Vermonters

Existing community nurse programs may apply to participate; successful applications must include plans to create a community health worker position, expand nursing services, and ensure home visits are not limited in number or duration

Participating programs must identify coverage gaps, integrate enhanced administrative support, and develop data collection systems to document needs, outcomes, and processes

Programs must submit a report to the Department by December 15, 2027 detailing the number of aging Vermonters served, program gaps, and recommended improvements

Effective date: July 1, 2025
