Authorizes the State Registrar to correct or amend a death certificate to reflect the deceased person's gender identity at the time of death

Applications may be submitted by the certifying health care professional, medical examiner, hospital, nursing home, or by family members including spouse, adult child, parent/guardian, adult sibling, or adult grandchild

Requires the Department of Health to adopt rules specifying acceptable evidence of gender identity (including advance directives), required forms and affidavits, and procedures for individuals to predesignate their gender identity

Directs the Department to establish procedures for resolving conflicting requests for gender identity corrections through evidence review

Effective date: July 1, 2025
