Large and small group health plans issued or renewed on or after January 1, 2026 must cover one or more prosthetic limbs and custom orthotic braces per limb when medically necessary for daily living, job activities, or physical activities like running, swimming, and biking

Coverage must include materials, components, related services, instruction on device use, and reasonable repair or replacement without continuous use or useful lifetime restrictions when medically necessary

Replacement or repair is covered when the patient's physiological condition changes, the device becomes irreparably damaged, or repair costs exceed 60% of replacement cost

Health plans cannot deny coverage for enrollees with disabilities if similar services would be covered for non-disabled persons seeking to restore the same physical activity

Does not apply to individual market plans or self-insured/fully insured large group plans for public employees and school employees under chapter 41.05 RCW
