Collaborative drug therapy agreements and their attachments/appendices are made confidential and exempt from public records disclosure under Washington's Public Records Act (RCW 42.56.360)

Agreements may be disclosed only when they form the basis of a disciplinary action resulting in misconduct findings against a licensed healthcare provider

When disclosed for disciplinary purposes, personally identifiable information of individuals other than the disciplined license holder must remain confidential

Personally identifiable information is defined as names, mailing addresses, credential numbers, email addresses, practice site facility addresses, and phone numbers
