Creates a mandatory annual reporting requirement for health care entities beginning June 30, 2027, requiring disclosure of ownership structures, private equity involvement, management services organizations, governing board compensation, and financial reports to the Department of Health

Defines "control" as ownership of 10% or more of voting shares or ability to direct actions/policies, and covers health care providers, facilities, carriers, benefit managers, and provider organizations

Requires the Department of Health to develop a public interactive search tool by January 1, 2028 displaying entity ownership, affiliations, and trends in horizontal and vertical consolidation

Exempts independent provider organizations with two or fewer providers, and entities whose parent company reports on their behalf

Establishes civil penalties up to $50,000 for small entities (10 or fewer physicians or under $10 million revenue) and up to $500,000 for larger entities for failure to report or submitting false information
