Requires organizations failing to meet more than half of agreed performance targets to develop remediation plans with revised thresholds to demonstrate progress on service changes

Terminates contracts and state funding for one year for organizations that fail to achieve agreed progress under remediation plans, requiring review of alternative delivery strategies during termination

Sets funding allocations at up to 90 cents per capita for urban counties (capped at $300,000 per organization) and $40,000 base plus 90 cents per capita for rural counties

Requires the Department of Commerce to submit biennial performance reports to the legislature by December 31st of each even-numbered year
