Requires health insurance policies and self-insured governmental health plans to cover diagnosis and treatment for infertility, including at least 4 completed egg retrievals with unlimited embryo transfers following American Society for Reproductive Medicine guidelines

Mandates coverage of standard fertility preservation services for patients undergoing medical treatments (such as chemotherapy, radiation, or surgery) that may impair fertility

Defines infertility as failure to establish pregnancy after 12 months of unprotected intercourse (6 months if woman is 35 or older), inability to reproduce without medical intervention, or based on physician findings

Prohibits insurers from imposing different exclusions, limitations, cost-sharing, or waiting periods on infertility services compared to other covered medical services, and bars restrictions based on participation in third-party fertility services (such as surrogacy)

Applies to disability insurance policies, defined network plans, preferred provider plans, and self-insured plans of state and local governments; takes effect on the first day of the 4th month after publication
