Creates a written affidavit process allowing mothers, fathers, or alleged/presumed fathers to disclaim parental rights for children under age one without appearing in court, provided the child is not an Indian child

Establishes revocation windows: fathers can revoke within 72 hours of execution or 120 hours after birth (whichever is later); mothers can revoke within 72 hours of execution; after these periods, disclaimers become irrevocable unless obtained by fraud or duress

Requires adoption placement agencies to offer counseling to all parents and legal counseling to minor parents at the agency's expense before the affidavit is executed

Prohibits courts from granting adoption orders until 3 months after the disclaimer affidavit was executed, and bars legal challenges to the affidavit after 3 months from execution

Allows payments to out-of-state private child placing agencies licensed in their home state for adoption services, expanding current law which only permits payments to Wisconsin-licensed agencies
