Requires defined network plans, preferred provider plans, and self-insured governmental plans to cover out-of-network emergency ambulance services at rates determined by: (1) mutual agreement, (2) local government-approved rates, or (3) 350% of Medicare rates or the provider's billed rate, whichever is less

Prohibits health plans from imposing higher cost-sharing on enrollees for out-of-network emergency ambulance services than would apply for in-network providers

Prohibits ambulance providers from balance billing enrollees beyond their required copayments, coinsurance, or deductibles once reimbursed at the applicable rate

Requires insurers and self-insured health plans to respond to emergency ambulance claims within 30 days by either remitting payment directly to the provider or notifying the provider of claim deficiencies

Excludes air ambulance services from the emergency ambulance coverage requirements; takes effect on the first day of the 4th month after publication
