Requires the Department of Transportation to ask applicants for driver's licenses or state ID cards whether they want to provide contact information for up to 3 individuals for whom they serve as primary caregiver

DOT must record any submitted caregiver contact information in the applicant's file and make a separate form available on its website and at local examining centers

Law enforcement agencies can electronically request and automatically receive caregiver contact information along with any emergency contact information from a person's DOT file

DOT is prohibited from disclosing caregiver information to anyone except the applicant or law enforcement agencies

Applies to original, duplicate, reinstated, reissued, and renewal license or identification card applications
