Applies to limited service health organizations, preferred provider plans, and defined network plans in addition to standard disability insurance policies

Allows insurers to charge less than $35 or impose no cost sharing on insulin; does not mandate any minimum cost sharing

Takes effect on the first day of the 4th month after publication
