Creates state criminal penalties for using force, threats, or physical obstruction to injure, intimidate, or interfere with persons obtaining or providing reproductive health services, or exercising religious freedom at places of worship

First violation is a Class A misdemeanor (up to $10,000 fine and/or 9 months imprisonment); second or subsequent violation is a Class I felony (up to $10,000 fine and/or 3.5 years imprisonment)

Prohibits intentionally damaging or destroying property of reproductive health facilities or places of religious worship

Establishes civil cause of action allowing aggrieved persons to seek injunctive relief, compensatory and punitive damages, attorney fees, or statutory damages of $5,000 per violation

Authorizes attorney general or district attorneys to bring civil actions with forfeitures ranging from $10,000 to $25,000 depending on whether the violation was a first or subsequent offense and whether it involved nonviolent physical obstruction
