Requires health insurance policies and self-insured governmental health plans to cover diagnosis and treatment for infertility, including at least 4 completed egg retrievals with unlimited embryo transfers

Mandates coverage of standard fertility preservation services for patients facing medical treatments that risk impairing fertility (chemotherapy, radiation, surgery, etc.)

Defines infertility as failure to establish pregnancy after 12 months of unprotected intercourse (6 months if woman is 35+), inability to reproduce without medical intervention, or based on physician findings

Prohibits insurers from imposing different exclusions, cost-sharing, waiting periods, or benefit limits on infertility services than those applied to other covered medical services

Applies to state and local government self-insured plans and takes effect the first day of the 4th month after publication
