Requires the Department of Health Services to request a federal Medicaid waiver to provide prerelease health coverage to incarcerated individuals for up to 90 days before their release

Covered services include case management, medication-assisted treatment for all substance use disorders, and a 30-day supply of prescription medications

Applies only to incarcerated individuals who are otherwise eligible for Medical Assistance (Medicaid)

Upon federal approval, DHS must reimburse both the federal and state share of covered services to the extent authorized by the waiver

Waiver request must be submitted to federal HHS by January 1, 2027
