Department of Transportation must ask driver's license and ID card applicants if they want to provide names and contact information for up to 3 individuals for whom they serve as primary caregiver

Caregiver contact information forms must be available on DOT's website and at all local examining centers

DOT must record submitted caregiver information in the applicant's file and provide law enforcement agencies with electronic access to automatically retrieve this data when identifying a person

Disclosure of caregiver information is restricted to the applicant themselves or law enforcement agencies only

Applies to all original, duplicate, reinstated, reissued, and renewal license or ID card applications
