Authorizes the West Virginia Department of Environmental Protection to promulgate nine legislative rules covering hazardous waste management, ambient air quality standards, new stationary source performance standards, hazardous waste facility air pollution control, hazardous air pollutant emissions, water quality standards, underground injection control, and water pollution control permit fees

Establishes Category A as a non-federally-protected water use designation that can be removed from surface waters upon demonstration that the water body lacks capacity to support public water supply use, subject to EPA approval and a 45-day public comment period

Sets Ohio River main stem fecal coliform limits at 2,000 per 100ml as a monthly geometric mean during the non-recreational season (November through April), based on at least 5 samples per month

Modifies underground injection control permits to allow Class 2 and 3 wells permits for the operating life of the facility and Class 6 wells for operating life plus post-injection site care, with Director review every 5 years

Requires the Department to issue draft NPDES permits and fact sheets at least 3 months before permit termination, provide permittees 10 business days to review drafts before public notice, and meet with permittees upon request to discuss draft permits
