The $500 annual cap continues each year and only ceases upon the death of the insured

Maintains existing requirements that therapy copayments cannot exceed those charged for primary care physician or osteopathic physician services

Applies to all accident and sickness insurance policies, provisions, contracts, plans, or agreements in West Virginia
