Requires the Department of Human Services to establish an Insurance Buy-In Plan by January 1, 2026, available to West Virginia residents who are ineligible for Medicaid and Medicare

Covers essential health benefits including hospitalization, emergency services, mental health and substance use disorder treatment, prescription drugs, preventive care, and pediatric services including dental and vision

Establishes a sliding scale for premiums and cost-sharing based on household income, with discounts required for residents earning below 200% of the federal poverty level

Requires the department to apply for federal Basic Health Plan Option (Section 1331) and/or State Innovation Waiver (Section 1332) to maximize federal funding for the program

Prohibits employers from disenrolling or denying enrollment in employer-sponsored health coverage based on an employee's potential eligibility for the buy-in plan
