Mandates insurance coverage for one baseline mammogram for women ages 30-39, annual mammograms for women 40 and older, and mammograms at any age when medically necessary for women with family history or other breast cancer risk factors

Requires coverage for comprehensive ultrasound screening when mammograms show heterogeneous or dense breast tissue, or when women are at increased risk due to family history, prior breast cancer, or positive genetic testing

Applies coverage requirements to public employee insurance, accident and sickness insurance, group insurance, hospital/medical service corporations, health care corporations, and health maintenance organizations

Requires FDA-certified mammography facilities to notify patients of their breast density classification using the American College of Radiology's Breast Imaging Reporting and Data System, including information on potential risks and benefits of additional screening

Removes mammograms from existing third-party reimbursement provisions that tied coverage to United States Preventive Services Task Force guidelines, replacing with the new expanded coverage standards effective January 1, 2025
