Creates the Minority Health Advisory Team (MYHAT), a 20-member body appointed by the Commissioner of the Bureau for Public Health by August 1, 2025, to advise on public health services for minority populations

Establishes a Community Health Equity Initiative Demonstration Project effective July 1, 2025, running for four years until July 1, 2029, to develop model programs addressing social determinants of health

Eligible communities must have approximately one square mile area, at least 3,000 residents, with 25% minorities, 55% living in rental housing, and 50% with incomes under $27,000

Advisory team includes representatives from WVU and Marshall medical/public health schools, state agency commissioners, local health departments, hospitals, federally qualified health centers, and community members

Requires annual progress reports from participating communities starting November 1, 2025, and summary reports to the Legislative Oversight Commission of Health and Human Resources Accountability starting January 1, 2025
