Adds religious and philosophical exemptions to West Virginia's mandatory vaccination requirements for school and child care center attendance, allowing parents or guardians to submit a written statement declining immunizations based on their beliefs

Simplifies the medical exemption process by allowing a physician, physician assistant, or nurse practitioner to provide a written statement that immunizations are detrimental or not appropriate for a child, replacing the previous system requiring commissioner approval

Eliminates the State Immunization Officer position and removes requirements for schools to report unvaccinated children attempting to enroll

Prohibits schools and state-regulated child care centers from excluding children with exemptions from extracurricular activities or school-based events

Creates a private right of civil action allowing parties seeking or granted exemptions to sue for compensatory damages, punitive damages, and equitable relief if a school or child care center takes adverse action against them
