Initiation phase requires a comprehensive test upon entry, followed by qualitative testing every two weeks for six weeks

Stabilization phase requires qualitative testing every 45 days for one year

Maintenance phase requires qualitative testing every six months on an ongoing basis

Treatment providers may request additional qualitative or quantitative tests at any point to assess patient compliance
