Mandates three phases of supportive living arrangements: on-site independent living, scattered site living, and supportive independent living, with statewide network coverage

Establishes a per diem rate structure for transitional youth services that maximizes federal Title IV-E reimbursement until age 21, includes annual cost-of-living adjustments, and must be reviewed for sufficiency every three years

Requires services addressing practical daily living skills, social and emotional skills, and education and career development to help youth achieve self-sufficiency

Mandates annual reports to the Legislative Oversight Committee on Health and Human Resources beginning December 1, 2027, including data on housing stability, employment, education outcomes, and funding sources
